Title: What is the best way to recreate a land?

I have a question; what is the best way to recreate a specific land? I've been using a heightmap, and though that works great. Now I'm faced with a problem. I want to recreate the following landscape in Terragen 2:

But as you can see, the island at the front has a "hole" in it. How would I make something like that? I'd photoshop it, but this is the second project I've run into something like this.

Would suggest to use Terragen 3.1
It has displaceable objects that could be used for the arch.

Ulco "Dune" had already posted several works about it.

download the .tgd and mess around, good luck...it'll be a fine image.
