To answer your question, its all of the things you mentioned. Some images are completely painted from the start, either by hand , digitally in Photoshop or by a computer program such as terragen. Some images are done in Photoshop by photo manipulating various backgrounds pictures to create a "matte painting" background and then adding an aircraft which comes from a flight-sim game. Such as IL-2 Sturmovik or other cool Sims.
I often grab a screenshot from a previous flight in the games or an already posted image from the fans, then slowly cut out the aircraft from the scene, rework the entire airplane and its many details, even markings and camouflage to represent the actual real aircraft I need for my scene.
Recreating an entire aircraft is often done when its for a special artwork for a World War 2 veteran (or his family) , or for some websites which represent various WW2 (or other era) Air Force Groups or Squadrons.
I also paint aircraft skins for various Sim aircraft, which also helps to create the actual airplane I need for an aviation scene. The whole process takes a few hours to paint and create. Anything from 6 to 48 hours, even more if you want to paint in all the fine details into the scene.
I have many tutorials available in many other forums dealing with this type of art. You can also check some of those club houses if you are interested in doing some aviation art for yourself.

They have many talented artists and good friends over there and tons other cool aviation art images posted by everyone. Some folks even create aviation art using Terragen-2 clouds and its backgrounds. So TG-2 is a great way to start making backgrounds for such aviation scenes.
If some guys and gals are not too good at using Photoshop for post-work image manipulation to add an aircraft into the background images, I suggest adding them by using some of the free airplanes and other objects already available in many internet websites, such as Bazze´s airplanes.
