Logged in, you begin.  Logged out, you lose it.

I think it's a little bit different from that.  First, you do need to be logged in.  Afterwards, it adds the time between each new page you load to the total time.  If you go more than 15 minutes between a page load, it considers your session to be over, and won't add any more time until you start surfing again.
